$4.2 million health initiative rolled out in Haskell Heights

The Eau Claire Cooperative Health Center's new innovations health concept is empowering families in the 29203 zip code, which has one of the highest Diabetes-related amputee rates in the country.

The $4.2 million health initiative was rolled out Wednesday at a special press conference. It links Medicaid and Medicare patients to community health coaches and primary care physicians who make at home visits.

Funding for the home visits comes from a national grant and the Eau Claire Cooperative Health Center.

Doctors who work in the area says this is a needed step.

"This is really the first time that people that have had ongoing problems can have individual care in the privacy of their own home," said Dr. Stuart Hamilton, founder of the ECCHC.

Any adult child or pregnant woman living in the 29203 zip code already enrolled in Medicare or Medicaid and has not seen a primary care doctor in at least year can register for the program. It's designed to give folks the personal tools they need to take charge of their health.

"A system that is going to drive proactive managing chronic diseases and focused on creating health, not just one treating disease. That is what we are really about. That is real progress and that is when you know that healthcare is really changing for the better," said Dr. Laura Long.
